Once you have booked your event with DJ Louie Tunes, click here to complete my online Wedding Info Form.

Please do not submit your form to me until I call to schedule a meeting with you. I will contact you approximately four (4) weeks before your wedding reception to schedule a meeting with you and your fiance. At the meeting (preferably at your reception location) we will finalize all of the information on your form and answer any questions you may have.

Note: Once you start entering information on your form, make sure you click the SAVE button before leaving the page.

Returning clients access your saved Wedding Info Form by entering your phone number here and then click the button below to locate your saved Wedding Info Form. (Be sure to use the same phone number that you used when you originally filled out the form.)
